The Chickasha Enrollment Center is here to help you with your enrollment questions, including:

• FORGOTTEN LOGIN  - IF YOU HAVE FORGOTTEN YOUR LOGIN INFORMATION - PLEASE EMAIL enrollment@chickasha.k12.ok.us AND INCLUDE YOUR NAME AND EACH OF YOUR CHILDREN AND THE GRADE THEY WILL BE ATTENDING THIS FALL
• NEW STUDENT ENROLLMENT Includes: first time enrollment, students returning from another district or home school, and incoming Pre-K students. Any students who did NOT finish the previous year at CPS are considered "New Students".
• IMMUNIZATION INFORMATION from SDE - FOR SCHOOL ENROLLMENT, A PARENT OR GUARDIAN SHALL PROVIDE ONE OF THE FOLLOWING:

1. CURRENT, UP-TO-DATE IMMUNIZATION RECORDS; OR
2. A COMPLETED AND SIGNED EXEMPTION FORM.  Please click here for the SDE's exemption information.  
    

   Your online enrollment is not complete until either an up-to-date immunization record OR SDE approved exemption letter is uploaded.
